Pre-Registration Process

Semmes Murphey is proud to offer you a secure and convenient patient pre-registration experience. We will send you a web link via text message or email so that you can pre-register electronically prior to your appointment. Check-In Process

We are pleased to introduce our new registration station and check-in process. With this new system, we are working to create a better healthcare experience for you!

1. When you arrive for your appointment, simply walk up to one of our check-in kiosks in the connector area between our two buildings. Staff will be there to help you through the process. 

2. Please have your driver’s license and insurance card ready before you touch the kiosk screen.

3. The kiosk will ask you a series of questions and ask you to scan your driver’s license and insurance card. (You may put the cards back in your wallet after scanning them.)

4. The process the first time may take you a little longer, but the next time you visit, you will be asked only to verify your information and scan your cards. After you are done answering the brief questions, please take a seat and we will call you for your appointment.

Please review the following information before your appointment:

  • Arrive 30 minutes early for your appointment

  • Bring your current insurance card and photo identification (such as a driver’s license)

  • Bring a written list of medications with dosage you are currently taking, including vitamins and supplements

  • Bring a referral if required by your insurance carrier

  • Payment is due at the time of service, including any co-payments required by your insurance carrier

  • Patients under 18 must be accompanied by a parent or guardian or must have written permission to treat on file from the parent or guardian
